Aniello "Mr.Neil" Dellacroce, underboss of the Gambino Family for Carlo's reign and into Paul's! Mentor to "the Dapper Don", John Gotti Sr., should've been named boss upon Carlo's death, the Gambino family would be alot different if this was to have happened! Great man, he was Cosa Nostra!
He was Cosa Nostra through and through. No doubt. But he shouldn't have been boss. In spite of Castellano being a disaster as a boss, whether we like it or not, he had the smarts and the ability to manage and execute white collar crime. The 'Mafia's' business model was changing, Gambino saw that and knew that while appointing Castellano would been seen as favoratism and not the popular choice, he likely felt that he was younger than Dellacroce and had the time to transition the family to a more refined model. It didn't happen of course and what happened with Gotti was a disaster and exactly what Gambino wanted to avoid.
